Legal agreement For successful registration : By filling out and sending the registration form, a) I grant permission to SKI CLUB JIZERSKÁ PADESÁTKA as the organiser and to the partners listed on the website www.jiz50.cz in the section Partners (hereinafter referred to as the “Administrators”), in accordance with the relevant regulations of Act No. 480/2004 of Coll. in its valid reading, to use my mobile-phone number and email address, namely for business and marketing purposes and also for the purpose of being sent information on sporting events and competitions and being informed of the competition results; b) I grant permission to the Administrators, in accordance with the relevant regulations of Act No. 101/2000 of Coll. and Act No. 480/2004 of Coll. in its valid reading, to process my personal data listed in this registration form, for the purpose of verifying my valid participation in a sporting event (competition), being awarded a prize and for other business and marketing purposes of the Administrators and further for the purposes of being sent information on sporting events and competitions and being informed of competition results, all that for the time of ten years; also to publish them to the extent of: first name, surname, year of birth, the name of municipality and place of residence in the media and on the website www.jiz50.cz. At the same time, I express my agreement with these personal data being processed fully and for the abovementioned purposes also by a third party authorised by any of the Administrators, namely by the company Mather Activation s.r.o. The Administrators also have the right to add other personal data concerning my person to these personal data. I am providing the permission voluntarily. I have the right to withdraw my consent, to each of the Administrators separately in writing sent to the address of their headquarters, and also the right according to Section 21 of Act No. 101/2000 in Coll., i.e. especially the right to access the data concerning me, the right to complete them, correct them, block them or demand their liquidation, as well as the right for apology, monetary satisfaction or compensation if my right to human dignity, personal honour and good reputation or the right to the protection of my name has been breached by the conduct of an Administrator. A withdrawal of consent comes into effect on the moment of delivery and results in my being excluded from further participation in the competition (contest) for which I have registered, including the loss of a claim to be presented a prize if the withdrawal is delivered before its presentation. My personal data can be, in accordance with Section 5, Paragraphs 6 to 9 of Act No. 101/2000 of Coll. in the following extent: first name, surname and address, passed on to partners of the competition Jizerská 50, who are listed on www.jiz50.cz, in the section Partners. c) Should I win any prize which will be, according to these rules, presented to the winner in person, I grant to the Administrators, in accordance with Section 12 of Civil Code in its valid reading, permission to use my portrayal, my written expressions, pictorial depictions and video as well as audio recordings dealing with my person or my speeches of a personal character recorded in connection with the organisation of this competition (contest) and presentation of the prize (hereinafter only as the “Depiction”) for commercial use in all communication media regardless of their character and purpose, namely in all normal ways, I grant permission to their subsequent modification and possible connection with other works or incorporation into a collected work. I grant this permission free of charge, without a material, temporal, quantitative and spatial limitation, and I can withdraw it at any time. This permission is also valid for third parties who are provided the Depiction by the authorised subject in accordance with its purposes.
